​The following forms can be printed, completed, and dropped off at the office window or in the drop boxes located in the first entry or next to the office door in Lakeview Tower, mailed to MRHC, 111 S. Washington Avenue, Madison, SD  57042, faxed to 605-256-9677, or emailed to [email protected] .
​Application forms:  for Section 8 Housing or Public Housing in Lakeview Tower.  These applications have no waiting list and will be processed as soon as the application along with paperwork needed to begin the process are received.  The first page of the application lists these required items.  Applications cannot be processed without them, so the faster all paperwork is turned in, the faster the process will go.  Must be able to pass a criminal background check.
​Tenant Change form:  is used to report new income change (increase or decrease), termination of employment, household/family member changes, changes in childcare costs, or changes in medical care costs.
​Landlord Packet:  used to gather information for new Section 8 tenants or Section 8 tenants who are relocating/moving.
​Landlord listing:  Madison, Howard, Salem, and Montrose landlords and apartment complexes.  We serve Lake, Miner, and McCook counties in South Dakota.  
​Townhome Application form:  Townhome applications are considered upon order received.  When a townhome is advertised, there is a waiting list going until the unit is rented.  ​Must be able to pass a criminal background check and have income to pay rent on the unit per month plus utilities.
